If she’s the first since Tim, it’s just the two of them travelling the universe. Alone on a spaceship, and the Doc doesn’t have anyone else to torment, and the kid doesn’t have anywhere else to go to, and they probably trust her, and all the promises she’s made, no matter how many she’s already broken. Carmilla looks at him the way she used to just before she left– all the same sadness, all the same regret, but he’s always known she’d do it all again if given the chance. In which I ask, “How would Jonny respond to discovering that Doctor Carmilla made another Mechanism?” The answer is badly, but I think we all expected that.
